Advancing a Healthier Wisconsin Endowment Community Health Award

Acknowledges an organization working to eliminate health disparities through systems change. Demonstrated commitment to minimizing the negative impact of social determinants of health as defined by the US Department of Health and Human Services (Check out Healthy People 2030 for more information). Increasing awareness of and interventions to improve social and environmental factors contributing to health disparities. Innovation, collaboration, and investment. Focused on geographic area or population negatively affected by the social determinants of health.

NOMINEE QUESTIONS:
1. Please provide a brief description of the organization, including its history, long term goals, partnerships, and investment.

2. Describe how the organization has made significant progress regarding one or more of the following domains, including any relevant, available measures of impact. Education Access and Quality Health Care Access and Quality Neighborhood and Built Environment Social and Community Context.

3. How has the organization affected systems. structures, or conditions for a specific population or geographic area?

4. What is being done to sustain positive system change over time.

Applicants must apply for a new program or project if they have been named as a MANDI finalist within the last five years. Nominations for projects/programs that have been named as finalists in the last five years, will not be considered. Award applications will remain open through April 23rd, 2024 at 12pm (noon). These awards are intended to highlight low to moderate income neighborhoods within the city of Milwaukee. Primary impact area and population served must be within the City of Milwaukee limits. Completion of a MANDI application indicates affirmation and certification that all information and answers to questions herein are complete, true and correct to the best of your knowledge and belief.
